Co-pays are collected at the time of visit prior to seeing the provider.
Always bring your medication bottles to each visit so that they may be reviewed by our staff and compared to your chart for accuracy.
Please allow 7 business days for all prescription refill requests.
No medication samples are provided without an appointment. No antibiotics or pain medications are dispensed without an appointment.
Form completion requires a face to face visit. There is a $25 form fee.
